Job Description​

The Opportunity:

As a Home Hospice Social Worker, you are responsible and accountable for assessing the psychological, social, emotional, cultural and/or financial needs that impact patients’ health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.

Shift:  PRN-day shift

The Work:

In this Social Worker role, you will have a wide variety of interesting and challenging responsibilities to serve residents who are in the community’s skilled nursing center for rehabilitation.  Some of your specific responsibilities would include:

• Identifying and understanding the significant social and emotional factors underlying a resident’s illness
• Participating with the health care team to complete MDS’s and develop care plans
• Working with patients and their family members to understand and accept necessary treatment
• Communicating with insurance companies and obtaining treatment/stay authorizations
• Helping patients adjust to the temporary and permanent effect of illness
• Referring patients and family members to community resources when needed
