The Opportunity:
Saint Luke’s Home Care and Hospice is seeking a compassionate Registered Nurse Hospice Case Manager to serve the metro Kansas City area. You will have the opportunity to help patients control and direct their end-of-life care by providing peace of mind, dignity, and comfort on the patient’s terms wherever they call home.
Established in 1975, Saint Luke’s Hospice is one of the region’s first hospice programs and has provided compassionate, comforting care to patients and their families in Missouri and Kansas. Our goal is to help patients and their families manage their physical, emotional, and spiritual wellbeing so they can stay focused on spending quality time together.
The Work:
Your skills as a Registered Nurse will be challenged in a meaningful way as you will be responsible for:
Conducting a comprehensive initial and ongoing assessments of the patient’s physical, functional, social, and environmental needs.
Providing professional nursing care by utilizing all elements of the nursing process.
Educating patients and their families on disease process, self-care techniques, end-of-life care, and nutrition and dietary needs
Collaborating with the interdisciplinary team (physicians, social work, chaplain, other caregivers).
Providing care that prevents and alleviates suffering.
Maintaining complete and thorough electronic medical records meeting all system and regulatory requirements.
Missouri and Kansas RN License and valid driver’s license with good driving record required.
This incumbent will typically work from around 11:30 am to 8:00 pm Monday through Friday with some opportunity for flexibility.
