Job Description​

The Opportunity:

We are looking for a highly qualified and career-minded Licensed Practical Nurse (LPN) to provide skilled nursing care in patient’s home.  You would be responsible for following the physician ordered plan of care.  You will work closely with an interdisciplinary team of nursing and therapists as well as patients and their families.

Shift:  PRN

The Work:

• Administration of professional nursing services. Ensuring patients receive services in accordance with their care plan
• Provide wound and ostomy care
• Patient education on medications and disease management
• Obtaining lab specimens
• Demonstrating excellent customer service skills and professional demeanor at all times
