Job Description​

The Opportunity:

We are recruiting for an Advanced Practice Provider with our Hospital in Your Home Team

Shift Details:

Days, weekends, and holidays are expected. This role is currently day shift, 7am-5pm, 4 days a week. Schedules may fluctuate based on the needs of the practice.  Travel to home settings will be required and travel during inclement weather may be required.

The Work:

The Advanced Practice Provider works collaboratively with physicians, APP’s,  and other multidisciplinary teams performing the following tasks in any authorized setting, including the home setting. This also will include virtual visit management of patients from our Plaza location command center

  • The physicians expect APP to be highly engaged in patient care.
  • The APP will be responsible for the medical management of the Hospitalist patient population within the scope of an Advanced Practice Provider. 
  • Comprehensive knowledge base of common Internal Medicine disease processes seen in the acute hospital environment. 
  • Demonstrates basic science and up to date clinical knowledge.
  • Independently round on assigned patient panel, with direct collaboration with physicians.
  • Communicate effectively with other team members and patients.
  • Establishes a patient relationship characterized by mutual respect, empathy, and collaboration.
  • Gather objective and subjective data to develop differential diagnosis, assessment, and a plan of care. 
  • Perform accurate physical assessment and interpret diagnostic and radiologic studies, labs, and review consultative recommendations.
  • Place appropriate orders and consultations.
  • Utilizes prescriptive authority in accordance with state and federal laws and regulations.
  • Performs ongoing assessment of patient response to plan of care, updates plan of care as needed based on response.
  • Accurately documents progress notes, discharge summaries and H&P in timely manner, with appropriate billing.
  • Communicates effectively using appropriate terminology, format, and technology in accordance health system policies.
  • Responsible for pager call during shift.
  • Takes initiative for seeking opportunities to advance knowledge and experiences.
  • Flexibility with scheduled rotations/locations as needed to ensure patient needs are met.
  • Able to respond and adapt to rapidly changing patient conditions.
  • Displays positive leadership skills amongst the multi-disciplinary team.
  • Acquires new knowledge quickly and efficiently.
  • Displays effective and clear communication amongst the multi-disciplinary team, including close collaboration with physician, consultants and nursing team. 
  • The term Advanced Practice Provider encompasses Nurse Practitioners, Physician Assistants, and Clinical Nurse Specialists.
  • The employee will have successfully completed a Master’s degree (or higher level of educational program) and have met the requirements for the respective State Board(s) (Missouri and Kansas).
  • The APP will hold and maintain a national certification and licensure as required by State Boards and Saint Luke’s Health System, as well as other certification as obligatory for this role.
  • 2 years experience in acute inpatient care is strongly preferred.
  • New grads may be considered with prior acute inpatient experience an RN.
