Job Description​

The Opportunity:

We are hiring a Clinical Scheduling Coordinator/Nurse Care Coordinator at the Plaza with our Maternal Fetal Medicine Team.

The Shift:

This is a Monday through Friday day shift position working from 8 a.m. to 4:30 p.m.

The Work:

The Nurse Care Coordinator serves as a member of an interdisciplinary maternal fetal care team and collaborates with maternal fetal medicine specialists, APP’s, other nursing staff, patient service specialists and schedulers to screen, assess, evaluate, and determine the appropriate plan of care of high-risk maternal fetal patients.  The position promotes timely patient access to services, and efficient and appropriate scheduling of services to be provided.   The Care Coordinator serves as the clinical point person to schedulers, and as a liaison between the providers and patient/family members.  The Coordinator assists in the development of protocols, policies, and procedures in collaboration with medical providers and hospital leaders. 

Job Responsibilities:   

  • Triages incoming referrals, reviews records and coordinates optimal and timely patient scheduling of complex, high-risk obstetric patients.   Adheres to established protocols and guidelines to determine appropriate timing of visits, testing and consultation.
  • Under MFM direction, establishes, reviews and updates operating guidelines for timing of appointments, necessary testing and type of appointments needed based on gestational age, history and assessed health indications.
  • Develops and maintains protocols, policies, and processes in collaboration with hospital leaders and medical providers, and in accordance with SLH policies.
  • Serves as an interdepartmental clinical liaison for scheduling, and a point of contact for hospital staff, referring offices, physicians and APPs with timely resolution, communication and follow-through.
  • Provides education to staff regarding appropriate scheduling protocols, and promotes delivery of timely, high-quality, high-value services across the continuum of care.
  • Assists other nursing staff in the department, as needed.

Qualifications:

  • BSN required
  • Active RN licenses (MO/KS) required
  • BLS Certification required
  • Strong clinical background with a minimum of 2 years of RN experience required
  • Clinical knowledge in high-risk obstetric pathophysiology strongly preferred
  • Previous ambulatory nursing experience strongly preferred
  • Basic competency in Microsoft Office software (Word, Excel, Teams) required
  • High level of problem solving and critical thinking skills
  • Ability to communicate clearly, verbally and in writing required
  • Ability to adapt to the evolving and changing guidelines required
  • Position requires being organized and detail oriented
  • Ability to establish and maintain effective working relationships with the interdisciplinary team regarding the ongoing care and treatment of patients required
  • Ability to work in a culturally diverse environment effectively required
