Job Description
The Opportunity:
Saint Luke’s Home Care and Hospice is seeking a part or full-time Social Worker to join our home care and hospice team in Chillicothe, Missouri. As a hospice and home care Social Worker, you will play an important role in the care and psychosocial wellbeing of our patients while enjoying the autonomy of setting your own workday and spending more time with those you serve.
Shift: We are seeking a candidate who is able to work at least 32 hours (part-time) per week, but will convert this position to 36 or 40 hours per week for a candidate interested in full-time status.
The Work:
In this Social Worker role, you will have a wide variety of interesting and challenging responsibilities to serve both hospice and home care patients. Some of your specific responsibilities would include:
Providing continuum of social work services to Home Health patients.
Assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery.
Providing professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning.
Collaborating with members of health care team in the development of a transition plan and addresses needs in a timely manner.
Assists patients who have a terminal diagnosis, and their families, navigate the end-of-life journey.
Participating in program planning and development to meet the needs of customers within the health care system and community.

Applicable Experience:
