Job Description
Capacity Care Coordinator is a practicing nurse responsible for big picture focus of care transitions through the continuum of care for the SLHS 1,300 acute care beds from discharge to post-discharge access of care within SLHS. The incumbent will insure that post-care appointments with PCPs and specialists are established prior to discharge. The incumbent will report directly to the Chief Capacity Officer (CCO) and will work with the Capacity Triage Officer (CTO) / Discharge Officer to resolve barriers for safe discharge. An integral part of the multi-disciplinary team, the Capacity Care Coordinator is responsible for ensuring quality patient outcomes, monitoring resource utilization and mitigating obstacles to care delivery through interdisciplinary collaboration, communication and coordination. This includes developing and carrying out discharge plans and mitigation of barriers that impede care progression.
The Capacity Care Coordinator will collaborate with SLHS social workers and care progression coordinators to develop an individualized discharge plan. Identifies additional services needed and makes appropriate referrals.
Leads and facilitates multi-disciplinary rounds. Helps to ensure clinical readiness for discharge. Collaborates with the CCO to establish outlier meetings at all metro hospitals.
Collaborates with multidisciplinary team. Assists with throughput management and works to mitigate delays in care.
In collaboration with attending physician, and the CCO identifies appropriate length of stay and monitors resource utilization.
Works with patients, families and caregivers to ensure patient needs are met, and a timely and appropriate discharge plan is implemented.
Maintains appropriate documentation. This is a working position.
Develops relationships with medical directors, nursing directors, and liaisons at LTACs, skilled nursing facilities and nursing homes. Develops relationships with insurance representatives. Teaches physicians, social workers and care coordinators how to prioritize discharge needs.
Job Requirements
Applicable Experience:
3-5 yearsAccredited Case Manager - RN - National Board for Case Management (NBCM), Basic Life Support - American Heart Association or Red Cross, Registered Nurse - VariousBachelor's Degree
