Care Coordinator - Saint Luke's South - Full Time Day

Job Description​

As a registered nurse care coordinator on this team you will be responsible for the coordination of care for patients with various complex conditions, treatments, and care plans. You will manage patient care through the continuum of care and ensure quality of care and services. You will collaborate with a diverse multidisciplinary team to provide oversight of care, treatment plans and interventions. As an informal leader on this team you will be expected to be confident in your skills, ask questions, and hold the care team accountable for quality patient outcomes.


This is a solution focused, knowledge seeking, positive team that will go above and beyond to find the answers and overcome challenges. This is a collaborative team that works with various departments and builds bridges between teams to keep patient care moving forward and timely and according to the care plan. This is a tight knit team that supports each other and helps each other; while working autonomously in their roles.


Responsibilities:
•    Makes quality patient outcomes a priority.
•    Overcomes obstacles to delivery of care through interdisciplinary coordination and communication. 
•    Coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress. 
•    Ensures that an appropriate plan of care is initiated.
•    Monitors care progression to ensure quality outcomes and effective transitions of care. 
We are looking for experienced inpatient nurses to join this team. Case Management or Utilization Review experience is preferred, but not required. This opportunity will provide you the opportunity to obtain your ACM certification. To be successful in this position, you will need to have great communication skills, ability to develop relationships easily, strong teamwork and a passion for collaborating with diverse teams with different experience and backgrounds to ensure quality patient centered care is delivered. You will perform as the patient’s advocate and will need to possess confidence in your skills to make sure decisions are made that are best for the patient.
