We are looking for an experienced LPN who is a natural leader, knowledgeable on disease processes, enjoys building relationships with their customers and enjoys helping their peers succeed. We are seeking a highly skilled LPN who isn’t afraid of jumping in and is eager to expand their knowledge in a variety of areas in an effort to support both home care and hospice.
Responsibilities:
This position interacts with internal and external customers to ensure we meet our regulatory requirements and provide support in most efficient and positive manner possible. A few of these tasks include the following:
Provide telehealth monitoring, evaluating responses, vitals signs, interacting with patients and physicians. This includes back-up during the week to cover PTO, and monitoring every other weekend (this part can be done remotely). Provide telephonic support for physician and patient calls for both home health and hospice as needed. Perform support functions for home health and hospice: such as, organizing information for Hospice IDG, completing NOE, uploading documents to the EMR, and general administrative support. Support the intake team for both home health and hospice. Supply management back up for agency. Administrative support for operations team as needed. Hospice insurance verification back up
