Job Description​

We are hiring a full time RN Nurse Manager to support our Population Health Outreach team. The Population Health Team within the Saint Luke’s Physician’s Group (SLPG) supports programs that foster the delivery of an innovative integrated care model across the Kansas City Metro and beyond.  SLPG participates in one of Medicare’s new advanced alternative payment models known as Comprehensive Primary Care Plus (CPC+) and is rapidly engaging in other value-based reimbursement models. The Manager for Population Health Care Management provides oversight of day-to-day operations of the Care Management department, ensuring consistent implementation of evidence-based best practice interventions aimed at achieving efficiency, effectiveness, as well as patient, provider and employee satisfaction. 

This position reports to the Director for Population Health and works in close collaboration with other members of the SLPG Leadership team and the Population Health Team. The position is based at the Saint Luke’s System Office but will require frequent travel to the other Saint Luke’s Health System locations both metro and rural. This is a full time position working M-F 8 to 5.

•    Implements evidence-based, best practice Care Management strategies, processes and interventions
•    Develops and monitors Care Management team goals; provides ongoing feedback and coaching; conducts annual performance reviews; routinely evaluates staff’s clinical skills and technical comprehension
•    Ensures an atmosphere of open communication, teamwork, and empowerment to make informed decisions 
•    Collaborates with Director of Population Health in analyzing reviews, metric data, and chart audits and uses that information to identify opportunities for improvement 
•    Collaborates with Director of Population Health on overall budget for Care Management
•    Oversees Care Management position control and hiring of new staff 
•    Contributes to the clinical, quality and financial outcomes of the Population Health Department
•    Familiarity with value-based payment models
•    Supports projects related to the CPC+ and other payor related value-based care programs
•    Performs all other related duties as assigned by Director of Population Health
•    Demonstrates a measurable, organized, enthusiastic and outcomes-oriented approach

•    Candidates must have a minimum five (5) years of progressive experience in care management and/or population health management, including at least 2 years at a managerial level 
•    Bachelor’s Degree in Nursing
•    Strong project management skills, including organization of individual and teamwork; prioritization; problem solving; adaptability; flexibility; and attention to detail
•    Teamwork, including the ability to draw out diverse perspectives, consider problem resolution from multiple angles, and build upon the experiences of all team members to create solutions that are broadly supported
•    Exceptional judgment and ability to learn the needs of different components of the Population Health Team 
•    Excellent verbal and written communication and interpersonal skills
•    Experience working with cross-functional teams in complex organizations on complex programs
•    Excellent time management and prioritization skills to focus on multiple projects simultaneously, adapt to change and work independently
•    Strong communication and follow through skills with a focus on collaboration and customer service.
•    Facility and ease with the use of the electronic health record and basic analytics tools to support population health management
