Job Description
The Medication Safety Coordinator will lead the medication safety agenda for Saint Luke’s Hospital and is responsible for managing and improving medication use, medication safety, and the implementation of new medication safety strategies. The essential function is to monitor and maintain current safe practices while developing and implementing plans to improve safety processes in the system. The position is in charge of monitoring current practice, reporting results related to medication safety, developing recommendations, implementing new processes, retrospective review of medication events, and prospective identification of medication risk to enhance patient safety. The Medication Safety Coordinator will collaborate with the pharmacy directors, pharmacy team, nursing management, physician staff, and other healthcare professionals to maintain and improve medication safety processes. Will function as Pharmacist approximately 20% of the time to maintain competencies.
Create a system vision for the safe use of medications in the organization.
Chair and construct the medication safety agenda to analyze and lead medication safety changes within the system.
Conduct prospective identification and retrospective analysis of medication risk, medication events, adverse drug events, and technology related safety errors to reduce system based errors.
Confirm operations reflect current medication safety best practices.
Address potential vulnerabilities in the current system.
Develop reports to trend medication incidents and perform analysis on near-miss medication occurrences.
Monitor safety errors related to infusion pumps, barcode utilization, and other technology that affects patient care.
Collaborate with interdisciplinary departments, including front-line employees, to prioritize safety issues and implement safety changes which create continuous quality improvement within the system.
Cooperate with interdisciplinary departments to implement new processes.
Measure and ensure process changes reflect medication safety outcome improvements.
Provide education, training, and continual communication to the pharmacy directors, pharmacy team, nursing management, physician staff, and other healthcare professionals.
Cover new safety initiatives, medication errors, adverse drug events, and safety concerns.
Supply departments with reports or graphs to provide continual feedback of new safety initiatives.
Review all medication policies and procedures to ensure they are in compliance with best practices. Review all new order sets, protocols, and clinical changes that are related to medication safety.
Ensure medication safety compliance is in line with The Joint Commission, DEA, FDA, Missouri Board of Pharmacy, and other pertinent agencies.
Employ, manage, and measure a Just Culture.
Oversee the collection of medication errors and events.
Remove barriers to reporting.
Seek feedback from periodic reviews of hospital policies and medication safety practices
Serve as an advisor and liaison between departments for medication safety.
Job Requirements
Applicable Experience:
3-5 yearsBasic Life Support - American Heart Association or Red Cross, Licensed Pharmacist (KS) - Kansas Board of Pharmacy, Licensed Pharmacist (MO) - Missouri Division of Professional Registration, Medication Therapy Services - Missouri Division of Professional RegistrationBachelor's Degree - Pharmacy
