Job Description
Saint Luke’s Hospital of Kansas City – Plaza location is seeking a Nurse Care Coordinator to join our team and work as the Inpatient RN Case Manager. In this role, you will ensure quality patient outcomes and overcome obstacles to deliver care through interdisciplinary coordination and communication. This care coordinator position works as the bridge between Nurses and Utilization Review and is responsible for the Nurse Case Management for their assigned unit.
This position leads multidisciplinary rounds, assists in identifying patients’ needs, assists with discharging and regulatory compliance. This is a fantastic opportunity to provide patients with education and connect them with the tools they need to be successful in their own care when they leave the hospital.
This role works collaboratively with the interdisciplinary team and partners with physicians to ensure patient treatment plan progress to ensure our vision, “The best place to get care. The best place to give care.” is maintained.
BSN Degree required within 3 years of start of position.
The Work:
Ensures that an appropriate plan of care is initiated
Monitors care progression to ensure quality outcomes and effective transitions of care
Supports and ensures the overall success of the patient’s healthcare by collaboratively working with patients, physicians, the interdisciplinary team, and health plans to select guide and direct patient care
We are looking for an experienced nurse who is passionate about advocating for their patients and providing patient education to help them understand how they can impact their own health and be part of the healing process. This is an autonomous role that is supported by a strong comprehensive team. We are seeking an RN who can see the big picture and will treat each patient uniquely to ensure individualized care is provided.
