Job Description​

Saint Luke’s Health System is currently seeking a Hospital in Your Home Inpatient Pharmacist to join our Hospital Pharmacy team. This new inpatient hospital concept involves inpatient hospital care delivered to the patient in his/her home.  This pharmacist will work in the command center with the virtual nurse and hospitalist to manage patients clinically and operationally through the respective SLHS hospital for which the patient is bedded.  All collaborative drug therapy management agreements and clinical monitoring for regular brick and mortar inpatients will apply.  Pharmacist is expected to prepare and participate in daily virtual patient rounds.  Pharmacist must have a good understanding of hospital medication supply processes.

Shift Details: Full Time

Various shifts covering weekday shifts as needed and a weekend(Sat & Sun) rotation every 3rd weekend.

Shifts are 8 hour shifts worked during the day.

The Work:

• Prepare and dispense medications.
• Collect organize and analyze patient and practice specific information.
• Initiate and monitor therapeutic plans.
• Work with command center nurse and hospitalist
• Coordinate medication supply with site inpatient staff
• Manage projects.
• Provide routine patient teaching and drug information services.

Individuals in this position will need to demonstrate the knowledge skills abilities and critical thinking skills necessary to practice pharmacy in a hospital and health system to be successful in this position. We are seeking a data driven, caring, patient focused, and safety minded pharmacist seeking an opportunity to grow their Pharmacist career. Hospital experience, residency, preferred, not required.

Job Requirements

Applicable Experience:

Less than 1 year

Basic Life Support - American Heart Association or Red Cross, Licensed Pharmacist (KS) - Kansas Board of Pharmacy, Licensed Pharmacist (MO) - Missouri Division of Professional Registration, Medication Therapy Services - Missouri Division of Professional Registration

Bachelor's Degree - Pharmacy
