Job Description​

Saint Luke’s Health System is currently seeking a Hospital in Your Home Inpatient Pharmacist to join our Hospital Pharmacy team. This new inpatient hospital concept involves inpatient hospital care delivered to the patient in his/her home.  This pharmacist will work in the command center with the virtual nurse and hospitalist to manage patients clinically and operationally through the respective SLHS hospital for which the patient is bedded.  All collaborative drug therapy management agreements and clinical monitoring for regular brick and mortar inpatients will apply.  Pharmacist is expected to prepare and participate in daily virtual patient rounds.  Pharmacist must have a good understanding of hospital medication supply processes.

Applicable Experience:

Less than 1 year

Licensed Pharmacist (KS) - Kansas Board of Pharmacy, Licensed Pharmacist (MO) - Missouri Division of Professional Registration, Medication Therapy Services - Missouri Division of Professional Registration

Bachelor's Degree - Pharmacy
