Job Description​

The Population Health Team within the Saint Luke’s Physician’s Group (SLPG) supports programs that foster the delivery of an innovative primary care model across the Kansas City Metro and beyond.  SLPG participates in one of Medicare’s new advanced alternative payment models known as Comprehensive Primary Care Plus (CPC+) and is rapidly engaging in other value-based reimbursement models. This job provides consultative, analytical facilitation and project management support for various Population Health strategic and special project initiatives.

This job reports to the Vice President for Population Health and works in close collaboration with other members of the SLPG Leadership team and the Population Health Team. The position is based at the Saint Luke’s System Office but will require frequent travel to the other Saint Luke’s Health System locations.
