As a Home Health Registered Nurse, you will function independently and provide patient care in the home setting. You will be responsible for care planning, case management, patient education, wound care, disease management and more in your patients’ home. You will work closely with your patients and their families. You will have the opportunity to work with a cutting edge remote patient monitoring system allowing you to perform virtual visits. As a nurse on our Home Health team, you will utilize your full scope of nursing skills, including wound and ostomy care, PICC line care, chronic care education and medication reviews. While you work autonomously, you will be supported by a multidisciplinary team of medical professionals and support staff.
Responsibilities
Missouri and Kansas RN License Required.
To be successful in this role, you must have strong critical thinking, time management, proactive total patient care, and organizational skills. You must be confident in your skills and able to work independently every day. You must be comfortable working with their patients in their home and willing to be flexible and adaptable with your daily scheduling and tasks. You must be able to stay positive in a changing environment with the ability to change directions quickly as needed.
A strong orientation team of leadership, educators and preceptors are available to you to ensure a successful transition into this position.
