Job Description

As a Hospice Nurse at Saint Luke’s Hospice House, you will provide end of life care and symptoms management to patients straight from the ICU outside their home. Many of these patients require aggressive symptom management and may be on drips, LVADs, ventilator/bipap, or have trachs. You will provide patient education to patients and patients’ loved ones, provide medication administration. You will work with a multidisciplinary team in the care management plan and use a team approach to provide exceptional patient care.

This is a self-motivated diverse team who are all confident in their skills and takes pride in what they do. They trust each other and help each other grow and provide exceptional care. They are proud to be part of the Hospice House and the Saint Luke’s Health System. Nurses on this team go above and beyond for their patients and their families. They practice autonomously and support each other.

Since 1975, Saint Luke’s Hospice—one of the region’s first hospice programs—has provided compassionate, comforting care to area patients and their families in their homes and nursing facilities. Because a person’s end-of-life needs may exceed the care available at home, Saint Luke’s recently opened its Hospice House and is accepting patients. Our skilled staff and volunteers handle patients’ needs so caregivers and family members can treasure the remaining time with their loved ones.

RN responsibilities include the following:

  • Provides direct nursing care to patients with terminal illness.
  • Management and coordination of care to include both the patient and family.
  • Coordinates the plan of care in cooperation with the interdisciplinary team.
  • Completes initial assessment of physical, emotional, spiritual and mental status.
  • Provides education and support related to end of life and imminent death.
  • Aggressive pain and symptom management, anticipating needs and administering ordered meds/treatments and/or obtains additional orders for improved comfort as needed.
  • Ensures safe and comfortable environment for patients and their families.
  • Open, sensitive, and non-judgmental presence accepting of different beliefs, cultures, and values.
  • Uses electronic documentation to describe symptoms, interventions, physical assessment, and education and support provided.

To be successful in this position, you will need to possess strong assessment skills, communication and organization skills. Must be passionate for providing the best patient care experience. We are seeking nurses who have strong time management skills and are able to prioritize tasks. We are looking for empathetic, compassionate nurses who are passionate about patient education and supporting patients’ loved ones.

Saint Luke’s has a strong nurse governance and we encourage all of our nurses to participate and help us make Saint Luke’s The Best Place To Get Care, The Best Place To Give Care. Our leaders support and encourage their nurses to take their careers to the next level and participate in various projects and initiatives that directly impact patient care and the organization.

This position works on a PRN basis and may include day, evening/night or weekend shifts.

Saint Luke’s Hospice House is a freestanding facility that offers hospice care for patients who may require more symptom management than can be provided at home or may prefer to spend their final days in another safe, loving location.
