Responsible for ensuring quality patient outcomes and overcoming obstacles to delivery of care through interdisciplinary coordination and communication. This position coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress. The Care Coordinator will ensure that an appropriate plan of care is initiated and will monitor care progression to ensure quality outcomes and effective transitions of care. BSN Degree required within 3 years of start of position.
Patient care coordination in the Cardiometabolic Center (patients with heart disease and diabetes) - RN triage, RN closer, patient navigation and coordination of care.
