Saint Luke’s Primary Care Clinics is hiring for a Nurse Care Coordinator for their Population Health team. This Registered Nurse will provide care to a diverse patient population. This role works collaboratively with multi-disciplinary teams to coordinate care and provide patient education to patients and their loved ones. This Nurse position will be responsible for treating the whole patient and assist our patients in becoming more independent and take ownership of their health and disease management.
This position will assist with clinical performance improvement activities and partner with external and internal partners to ensure we provide care to the total patient. This is a strategic RN position that will require you to build strong relationships with patients, care teams, and other internal and external partners predominately over the phone.
Hours: Monday - Friday, 8am - 5pm
