Saint Luke’s Primary Care Clinics is hiring for a Nurse Care Coordinator for their Population Health team. This Registered Nurse will provide care to a diverse patient population. This role works collaboratively with multi-disciplinary teams to coordinate care and provide patient education to patients and their loved ones. This Nurse position will be responsible for treating the whole patient and assist our patients in becoming more independent and take ownership of their health and disease management.
This position will assist with clinical performance improvement activities and partner with external and internal partners to ensure we provide care to the total patient. This is a strategic RN position that will require you to build strong relationships with patients, care teams, and other internal and external partners predominately over the phone.
Hours: Monday - Friday, 8am - 5pm

We are looking for an experienced nurse who is passionate about advocating for their patients and providing patient education to help them understand how they can impact their own health and be part of the healing process. This is an autonomous role that is supported by a strong comprehensive team. We are seeking a RN who can see the big picture and will treat each patient uniquely to ensure individualized care is provided. No two patients are the same.
Responsibilities:
Drive engagement strategies for improving physician and staff patient care practices based on clinical guidelines, best practices internal and external benchmarks, and claims-based data.
Supports and ensures the overall success of the patient’s healthcare by collaboratively working with patients physicians practice teams and health plans to select guide and direct patient care while integrating key features of the medical home.
