Social Services Coordinator - Bishop Spencer Place - Part-time

Job Description​

We are looking for a highly qualified and career-minded Social Services Coordinator to become part of the Bishop Spencer Place team and provide social work services to residents throughout our community with a focus on independent and assisted living. This is a part-time position, 30 hours per week, health benefits eligible, with some flexibility in work hours.

As a Social Services Coordinator you will be part of a strong supportive team working together to ensure exceptional resident care. You will have the full support of leadership and will be empowered to ensure everyone is working together to keep our residents first. Bishop Spencer Place has a resident centered care philosophy and we are looking for candidates who share those same values.

In this role you will have a variety of interesting duties, which may include:

  • Identifying and understanding the significant social and emotional factors underlying a resident’s illness
  • Participating with the health care team to complete MDS’s and develop care plans
  • Working with residents and their family members to understand and accept necessary treatment
  • Communicating with insurance companies and obtaining treatment/stay authorizations
  • Helping residents adjust to the temporary and permanent effect of illness
  • Referring residents and family members to community resources when needed
  • Evaluating residents for potential return to home or transfer to a more appropriate level of care
  • Coordinating the resident discharge process
