As a Home Infusion Registered Nurse, you will function independently and will be responsible for providing patient education and care in the home and inpatient setting. You will work closely with your patients and their families.
You will work autonomously with a collaborative and supportive team. You will interact with highly skilled individuals throughout the health system. Teamwork is an important aspect of this position to ensure we provide the best care possible to our patients.
Shift: Part-time
The Work:
Primary role of the home infusion nurse is to provide support for the product lines and service offerings for the home infusion pharmacy.
Establish and maintain positive working relationships with care coordinators and other healthcare providers to ensure a smooth transition of services
Establish clinical rapport with patients and other caregivers while providing education on services or prescreening of potential services
Communicate effectively with internal personal about any patient status changes or information that may affect the patient’s ability to receive treatment successfully
Complete all required clinical documentation at the completion of each service, including plan of care
Infuse medication in patient’s home according to physician orders. This may include IV placement, phlebotomy, infusion of medication via peripheral line, PICC line, or implantable port
Provide assessment of patient during infusion including, monitoring of vitals, monitor effectiveness of medication, manage side effects of infused medications to support a successful infusion, and provide education to patient and other caregivers (Subject matter expert for patient education, assessment, interpretation, and documentation of patient care and testing.)
Work with other home infusion nurses to provide patient follow up on new patients or new therapies initiated
Assist with all licensures and accreditations for the home infusion pharmacy
Assist in building patient education material and maintain all materials in accordance with accept nursing practices
Ability to work a flexible schedule to accommodate hospital discharges and patient schedules
Ability to be part of a rotating schedule to help support weekend discharges as needed
In times of low volume may assist in other areas including home health
Excellent customer service skills and professional demeanor at all times
Work with supervisor and other nurses to ensure all shifts are covered
Provides nursing care in compliance with the Nurse Practice Act.
To be successful in this role, you must have strong critical thinking, time management, proactive total patient care, and organizational skills. You must be confident in your skills and able to work independently every day. You must be comfortable working with their patients in their home and willing to be flexible and adaptable with your daily scheduling and tasks. You must be able to stay positive in a changing environment with the ability to change directions quickly as needed.
