The Opportunity:

Saint Luke’s Bishop Spencer Place has a career opportunity for a Social Worker (or Social Services Coordinator) to work in our skilled nursing and rehab unit.    

Shift:  This is a full-time position, health benefits eligible, with some flexibility in work hours.

The Work:

This is a rewarding and fast-paced position that includes the following responsibilities:

• Identifying and understanding the significant social and emotional factors underlying a resident’s illness.
• Participating with the health care team to complete MDS’s and develop care plans.
• Working with residents and their family members to understand and accept necessary treatment.
• Communicating with insurance companies and obtaining treatment/stay authorizations.
• Helping residents adjust to the temporary and permanent effect of illness.
• Referring residents and family members to community resources when needed.
• Evaluating residents for potential return to home or transfer to a more appropriate level of care.
• Coordinating the resident discharge process.

A successful candidate for this position will have a master’s degree in social work and be a Licensed MSW in the State of Missouri.  We will also consider a qualified individual to work as a Social Services Coordinator if they have at least a bachelor’s degree in a related field and relevant work experience.
