Job Description​

We are hiring an RN Hospital Service Coordinator for the within the Saint Luke's Cardiology Clinic This position will be based at the Plaza location and will focus on coordinating care for hospitalized patients for the Cardiology Clinic hospital services. This includes the appropriate triage and assignment of incoming patients to care teams including the teaching service. The requirements of this role includes a work schedule of 7 AM to 530 PM.

Specifically, this position serves as the liaison between the Cardiology Clinic providers and the hospital staff and will assist the providers in discharge preparation including communication with the multidisciplinary team to address key issues at discharge, including patient family preparedness and follow up care post hospitalization. The orientation of incoming residents and fellows to the cardiology service in another key responsibility of this position.

This position is accountable for utilizing the nursing process. Responsible for coordinating the plan of care for a group of assigned patients and consulting with other members of the health care team when indicated to ensure optimal patient outcomes.

This position may also be responsible for telephone triage, patient flow, and coordination of patient care. Coordinate emergent need patient appointments, identifies learning needs for patients and families, provide advice and recommendations, inserts intravenous access with medication administration and duties as assigned based on clinic need. Delegates tasks to clinical staff in accordance with demonstrated competencies and consideration to Scope of Practice. Subject matter expert for patient education, assessment, interpretation, and documentation of patient care and testing. Enters admission orders as directed by provider. Responsible for managing expired samples.

Able to work in a team atmosphere as well as autonomously. Excellent customer service skills and professional demeanor at all times. May be responsible for covering Medical Assistant job duties, LPN job duties, or other office duties as needed.

Job Requirements:

2+ years experience preferred

KS and MO RN Licensed required
BLS required

KS & MO RN License required; if the ideal candidate does not possess dual licensure at time of offer, he/she may be given an offer contingent upon completion of dual licensure within 90 days of start. BLS required.
